We at Great Mines Health Center are delighted that you have entrusted our organization with your medical, behavioral health and/or dental needs. We want you to have the best possible encounter each and every time. To ensure a positive experience, we would like to share the following information:

New Patients

  • Please arrive at least 30 minutes early to allow plenty of time to register and make any needed changes to your personal and/or insurance
  • You will be required to bring your photo ID and insurance cards (if you have insurance) at the time of your visit.
  • If the new patient is a minor (under the age of 18), a parent must present a photo ID as well as the new patient’s insurance information (if any). If the minor is adopted or in foster care, current guardianship/foster care documents will need to be presented at the time of service and be kept on file.
  • Patients must bring a complete written list of current medications, including dose and directions, so that your provider will know what you are already taking. Any prior medical documents should be presented to the provider at the time of visit.
  • • When bringing in paperwork to be filled out, such as sports forms or physicals, please complete as much information as possible prior to the visit.

Medication Refills

  • If you need refills on your medications, please call your pharmacy to request the refill.
  • If you have NO refills left on your prescription, please contact your pharmacy and THEY will contact us for a refill authorization.
  • Some medications may require a visit prior to providing a refill. Our staff will be happy to inform you if a visit is required.

Returning Patients

  • Please arrive at least 15 minutes early to allow plenty of time to register and make any needed changes to your personal and/or insurance information
  • You must bring your photo ID and insurance cards (if you have insurance) to each visit, as well as an updated medication list. This also applies to minor children’s office visits.
  • When calling to schedule an appointment, please provide as much information as possible so the proper amount of time can be allotted for your visit.
  • When bringing in paperwork to be filled out, such as sports forms or physicals, please complete as much information as possible prior to the visit.
  • Please let the provider know if a work/school excuse will be needed prior to the end of the visit.

Phone Calls

  • Potosi Office: Our office is open from 8 am to 6 pm Monday through Friday for the medical and behavioral health departments, and Monday through Thursday from 7 am to 5 pm for our Dental department.
  • Farmington Office: Our office is open from 8 am to 6 pm Monday through Thursday for all departments.
  • Phone calls will be returned within 24 hours of receipt of message.

Discounted Rates/Slide Program

The Slide Program fees for clinic services are based on your income and family size and may be reduced if you live on a limited income according to federal guidelines. We have A Sliding Scale Specialist available to answer any questions and provide information on required documents.
